One Health Community

One Healthy Community is a locally run, independent, non-government organisation that aims to proactively shape a healthier community by working in collaboration with primary health care providers and the broader community. The organisation was previously known as Fremantle Medicare Local.

One Healthy Community is building on the achievements of Fremantle GP Network and Fremantle Medicare Local through extending and strengthening engagement and support for general practice whilst building new relationships with other primary health care providers.
